6/29/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C greeted Ashiyah upon arrival to the home setting. Upon arrival to the home setting, Ashiyah appeared to be in a content mood. QMHP-C inquired about her day. QMHP-C informed Ashiyah that this session was going to focus on reducing angry and verbal outburst. 2. Behavior: Ashiyah actively participated in the session as she was cooperative throughout the session. 3. Behavior: Ashiyah participated in the session without incident. Ashiyah appeared to be sad as she was reporting to QMHP-C on her mother is getting on her nerves now they have moved. 1.Intervention: QMHP-C began the session by inquiring about Ashiyahâs current thoughts, feelings and emotions. QMHP-C provided Ashiyah with the opportunity to express her thoughts and feelings accordingly as she appeared angry. QMHP-C processed with Ashiyah about her feelings and reasons for her frustrations in which she chose to deny. QMHP-C used a feelings chart to assist Ashiyah with appropriately recognizing her feelings of anger. 2. Intervention: QMHP-C began processing with Ashiyah regarding her mother's perception of her behaviors. QMHP-C provided thought-stopping examples with Ashiyah to avoid reacting with aggressive behaviors toward authority and peers. QMHP-C discussed journaling with Ashiyah and the ability to gain insight into her own feelings. 3. Intervention: QMHP-C engaged Ashiyah in writing for instances jotting down a couple of feelings, contacting QMHP-C, and engaging in positive self-talk. QMHP-C provided Ashiyah several ways positive self-talking can help her get through difficult moments. QMHP-C role-played with Ashiyah different techniques to demonstrate appropriate ways to show feelings. QMHP-C openly discussed the importance of expressing feelings of depressive moods and not holding in feelings inside as she then will explode in a negative way when time presents itself. 1.Response: Ashiyah appeared to be receptive to QMHP-Csâ interventions and services that were provided to her during the session. During the session, Ashiyah was observed by QMHP-C as she distracted and upset. When asked she denied being angry and upset even though her facial expression displayed a frown. She was resistant to the therapeutic approach. 2.Response: Ashiyah also discussed with QMHP-C that she has been in arguments with her friends due to them wanting to start trouble amongst her and others and that is something that she is not going to allow to happen. Ashiyah verbalized to QMHP-C that she is trying to control herself but these âlittleâ children are going to make her act out of her character. 3. Response: Ashiyah appeared to be receptive to QMHP-Csâ interventions and services that were provided to her during the session. Ashiyah was able to identify feelings and thoughts since the last session as gave QMHP-C examples of situations that took place in the home that caused her moods to change.
